CASE MANAGEMENT Beginning in 2011, we are excited to offer our Case Management Service, a new initiative of PARAQUAD SA that will complement our existing services offered to people with spinal cord injury.
Case Management is a collaborative process of assessment, planning, coordinating, monitoring and evaluation of the services required that respond to a person's individual needs following spinal cord injury.
The steps that are involved in this process include:
Assessment
Assessment is the process that underpins all subsequent case management actions. It is an ongoing process of gathering and appraising information from various sources to identify areas where efforts will be focused. Assessment identifies gaps across the life domains.
Planning
The goals are agreed upon and are documented based on the individual's identified needs. Actions are planned, agreed upon and a timeline for review is set.
Coordination
This process entails linking the individual with agreed service providers, and coordinating these services for the benefit of the individual, if that is required.
Monitoring
The Case Manager will ensure that the individual receives the services, supports and resources that have been arranged to meet the individual's goals that have been documented in the plan. The Case Manager will also monitor the quality of the service(s) provided and work with the individual and service provider(s) to make any adjustments necessary for the individual to achieve their goals.
Evaluation
The Case Management Services is outcome oriented, so periodic reassessment and evaluation of whether the individual's goals are being achieved is an essential ingredient.
PARAQUAD SA is actively involved in a continuous improvement cycle utilising the Service Excellence Framework as our quality assurance mechanism.
Staff Member Providing The Service
Sharron Neeson will be the Case Manager responsible for delivering the Case Management Service. Sharron is a qualified social worker with a post graduate qualification in case management from Griffith University. She has been working for PARAQUAD SA since 1986 and has skills in:
- Providing counseling services to individuals and families;
- Project, financial and people management;
- Initiating and developing services
Sharron is prepared to travel to the client anywhere in South Australia and is a financial member of the Case Management Society of Australia.
